This Policy applies to our corporate website, located at https://www.all4cure.com/ (collectively, the “Site”), the All4Cure health tracking and patient networking platform (“Platform”) for cancers including multiple myeloma, and any other website, application, or services where we post or link to this Policy as well as offline services where we collect Personal Information from you (collectively, our “Service”).

This Policy does not apply to information processed by third parties, for example, when you visit a third-party website or interact with third-party sites, except to the extent those parties collect or process information on our behalf. General Location Data - Non-precise location data, e.g. location information derived from IP addresses.

Identity Data - Personal Information about you and your identity, such as your name or IP address, and whether you are a patient, researcher, or clinician.
Inference Data - Personal Information we create or use as part of a profile reflecting your preferences, characteristics, aptitudes, market segments, likes, favorites or your interests.
Race or Ethnic Origin Data - Information relating to your race, ethnic, or national origin.
User Content - Personal Information included in content provided by users of the Site in any free-form or unstructured format, such as in a “contact us” box, free text field, in a file or document, or messages to us.

Healthcare Providers -We collect Personal Information from your medical team, such as oncologists, other providers, and facilities, when you register and create an account, and otherwise use our Platform.
Caregivers - Caregivers or members of your family who may use the Platform on your behalf.
Automatic Collection - We may collect certain Personal Information automatically. For example, we collect Device/Network Data automatically using cookies and similar technologies when you use our Site, access our Site, or when you open our marketing communications.
Service Providers - We receive Personal Information from service providers performing services on our behalf.
Data we create or infer - We, certain partners, and third parties operating on our behalf create and infer Personal Information such as Inference Data based on our observations or analysis of other Personal Information processed under this Privacy Policy, and we may correlate this data with other data we process about you.
Social Media Companies - We receive Personal Information from social media companies, such as Facebook or LinkedIn when you interact with us on that social media company’s platform (e.g. our company page or account) on or in connection with our Service.

When you use our Site, we automatically collect and process Identity Data, Inference Data, General Location Data, and Device/Network Data. You may also be able to register for an account, access the platform, enroll in marketing communications, and contact us through our Site.

Third parties have their own privacy policies and their processing is not subject to this Policy.
Account Registration
We process Identity Data, Contact Data, and Biographical Data when you register and create an account for our Platform and Service, whether as a researcher, healthcare provider, or patient. Additionally, when you register as a patient, we also process Race and Ethnic Origin Data and Health Data (including related Inference Data) that you provide, or allow your healthcare provider or caregiver to provide, to us.

We use this Personal Information to create and maintain your account, to provide the services you request, and for our Business Purposes. For patients, we may also use this Personal Information to track and visualize your past treatments, to evaluate prior and future courses of treatment, to see what options are available for your next treatment, and to connect you with clinical trial opportunities.
The Platform

Generally
We process Identity Data, Device/Network Data, Inference Data, and User Content when you use our Platform. When you use our Platform as a patient, we may also process Race and Ethnic Origin Data and Health Data, and other related Inference Data collected as part of your Account Registration.
We use this Personal Information to operate the Platform, to develop the dashboard, to provide the services you request, and for our Business Purposes. For patients, we may also use this Personal Information to provide you and your treatment team with options and insights, to connect you with clinical trial opportunities, and ultimately to aggregate knowledge across patients, caregivers, clinicians, and researchers. This may involve the creation of Inference Data and other data relating to your treatment and related Health Data.
Please note: As part of the Platform, we may make available by default deidentified information derived from your Personal Information (including Health Data) in order to show basic patient background, course of treatment, and outcomes. Additionally, to help link our members with promising treatment options, we may disclose certain de-identified information with clinical trial sponsors to determine if key elements of your treatment history and personal background are a good fit for clinical trials.
Community Forum
We process Identity Data, Inference Data, Contact Data, and any User Content you post (e.g. forum posts and comments) on the All4Cure Community Forum. If you provide it in your post or comment, we will also process Health Data, Audio/Visual Data, and Race or Ethnic Origin Data.
We primarily process this Personal Information as necessary to make the forum available, to moderate posts and community interactions, and for our other Business Purposes.
Your posts may be public, or reposted on our Platform. When you post content on our Service, it may be publicly available within the forum, or in some cases, may be reposted by other forum users.
Contact Us
When you contact us using a contact phone number, contact us box, or via email, for more information, to inquire about investing, or to ask for support, we process Personal Information such as Identity Data, Device/Network Data, and any Personal Information contained within any User Content you provide. We may also collect Audio Data if you contact us via phone and/or leave a voicemail. We use Identity Data, Contact Data, User Content, and Audio Data as necessary to communicate with you about the subject matter of your request and related matters. We may also process this Personal Information for our Business Purposes.
Social Media Posts
We process Identity Data, Contact Data, and User Content if you interact with or identify us on social media or communication Site (e.g. if you post User Content that engages with or tags our official accounts). We process this Personal Information for our Business Purposes.
Posts may be public, or reposted on our Service. Content you provide may be publicly available when you post it on our Service, or in some cases, if you reference, engage, or tag our official social media accounts or communications Site.
Informational or Marketing Communications
We may process Identity Data, Device/Network Data, and Contact Data in connection with email communications that we send in relation to our Service. For example, we may send informational email communications to you if you upload new medical records, request to learn more about treatment options, or in connection with our delivery of services that you request. Additionally, you may receive marketing email communications if you contact us, choose to receive them, or interact with us in way that allows us to send you those communications. We may automatically collect Device/Network Data when you open or interact with those communications so that we can better understand engagement with our communications. We may also process this Personal Information for our Business Purposes. You can opt out of marketing communications at any time as specified in Your Choices.
Feedback and Surveys
We process Identity Data, Contact Data, Preference Data, and User Content collected in connection with user surveys or questionnaires.

We process this Personal Information as necessary to respond to user requests/concerns, for our Business Purposes, and other legitimate interests. We may share Feedback/Survey data relating to third party partners with those partners, who may use it for their own purposes.

In addition to the processing described above, we generally process Personal Information for several common purposes in connection with our business. Please see below for more information regarding the purposes for which we process your Personal Information.

We process any Personal Information as is necessary to operate the Site, and as otherwise necessary to provide you with the information, features, and services you request.

We may analyze network traffic, device patterns, and characteristics, maintain and analyze logs and process similar Personal information in connection with our information security operations and related.

Anonymized Data; Aggregate Analytics
We may anonymize Personal Information that we process through our Service such that this information is no longer readily linked or linkable to an individual in order to build new products or services, to create data relating to healthcare/treatment outcomes, to publish papers, and to create other analytics or reports. Anonymized data is not linkable to identified or identifiable individuals and households and is therefore not subject to this Policy. We may also process Personal Information as necessary in connection with our creation of aggregate analytics relating to how our Site is used, the pages and content users view, and to create other reports regarding the use and performance of our Site, and other similar information and metrics. The resulting aggregate data will not contain information from which an individual may be readily identified.

Personal Information we collect may be shared with a variety of parties, depending upon the purpose for and context in which that information was provided. We generally transfer data to the categories of recipients or in connection with specific business purposes, described below.
Service Providers
In connection with our general business operations, product improvements, to enable certain features, and in connection with our other lawful business interests, we may share Personal Information with service providers or subprocessors who provide certain services to us, or process data on our behalf. For example, we may use third party hosting providers to host our sites or content, and we may disclose information as part of our own internal operations, such as security operations, internal research, etc.

Community Members
We may share Personal Information with members of our community if you register as a patient, as described further above. By default, we share Health Data in pseudonymous form. However, if you allow us to, or if you participate in our community forum, the Personal Information you disclose as part of a forum post, or permit us to make available through your account settings (e.g. your name or other Identity Data) may be made public to other members of the All4Cure community.
Medical Team
If you grant your medical team access to the Platform, your posts, or other account information (including via links to your healthcare records system), we may share certain Personal Information, such as your Health Data and User Content, with your medical team. Please note: healthcare providers are not subject to this privacy policy. Please review the notice of privacy practices from your healthcare provider to understand how they handle and protect your Personal Information.
Researchers
We may share certain Personal Information, such as your Health Data and User Content, with researchers in connection with our mission to break down the silos that separate researchers from clinicians, academic oncologists from community oncologists, and patients from scientists. In some cases, your Personal Information may be shared in pseudonymous form (e.g. associated with a unique ID as opposed to your name), whereas in other cases, your information will be anonymized prior to sharing (and will no longer be covered by this Privacy Policy).
Clinical Trial Sponsors
To help link our members with promising treatment options, we may disclose certain de-identified information with clinical trial sponsors to determine if key elements of your treatment history and personal background are a good fit for clinical trials. Further, with your authorization (or that of your caregiver or healthcare provider) we may share your Personal Information with the clinical trial sponsors or sites in connection with your further evaluation for or participation in a clinical trial.
Social Media Companies
We may share certain Personal Information with social media companies when you interact with All4Cure on social media. We may allow these third parties to operate through our Service, such as through a social media icon link on our homepage. We do not share Contact Data collected in connection with All4Cure’s promotional communications with these parties.
Partner Networks and Practices
We share certain Personal Information with partner networks, institutions, and oncology practices in connection with our mission to break down the silos that separate researchers from clinicians, academic oncologists from community oncologists, and patients from scientists. In some cases, your Personal Information may be shared in pseudonymous form (e.g. associated with a unique ID as opposed to your name), whereas in other cases, your information will be anonymized prior to sharing (and will no longer be covered by this Privacy Policy).
Corporate Events
Your Personal Information may be disclosed to a third party in the event that we go through a business transition, such as a merger, acquisition, liquidation, or sale of all or a portion of our assets. For example, Personal Information may be part of the assets transferred, or may be disclosed (subject to confidentiality restrictions) during the due diligence process for a potential transaction.
Affiliates
In order to streamline certain business operations and develop products and Site that better meet the interests and needs of our customers, we may share your Personal Information with any of our current or future affiliated entities, subsidiaries, and parent companies.

Verification of Rights Requests
We verify all rights requests to ensure that the individual making the request is authorized to make that request, to reduce fraud, and to ensure the security of your Personal Information. For example, we may require that you verify that you have access to the email on file in order to verify your identity. If an agent is submitting the request on your behalf, we reserve the right to validate the agent’s authority to act on your behalf.

We operate in and use service providers located in the United States. If you are located outside the U.S., your Personal Information may be processed in the U.S. The U.S. may not provide the same legal protections guaranteed to Personal Information in foreign countries. 12. Children
Our Service are neither directed at nor intended for use by persons under the age of 13 in the US. We do not knowingly collect information from such individuals. If we learn that we have inadvertently done so, we will promptly delete such Personal Information if required by law. Consumer Health Data Privacy Rights
Your Rights
Under the Washington State MHMDA and similar state health privacy laws, residents and natural persons whose Consumer Health Data is collected in certain states may have the following rights, subject to verification, exceptions, and limitations:
Right to Confirm/Access/Know: You have the right to (a) confirm whether we are collecting, sharing, or selling your Consumer Health Data, and (b) access such data, including a list of all third parties and affiliates with whom we have shared or sold the consumer health data and an active email address or other online mechanism that you may use to contact these third parties. We may have the right to charge for requests submitted more than twice annually or that are manifestly unfounded, repetitive, or excessive.

Right to Delete: You have the right to request deletion of the Consumer Health Data held by us and our affiliates, processors, contractors, and other third parties.

Right to Withdraw your Consent/Opt-Out: You may withdraw any consent you have provided at any time. The consequence of you withdrawing consent might be that we cannot perform certain services for you, including any services conditioned on your consent or choice not to opt-out (e.g. clinical trial reference or participation in certain features of the Platform).

How to Exercise your Rights
You may submit requests as follows (please our review verification requirements below).

You may call us at (425) 477-9021. You will be directed to leave a voicemail where you will provide your email address, phone number or address, along with your request.

You may send an email to privacy@all4cure.com with your email address, phone number or address on file, along with your request.
If you have any questions or wish to appeal any refusal to take action in response to a MHMDA rights request, contact us at privacy@all4cure.com. We will respond to any request to appeal within the period required by law.

Washington Residents: If your appeal is unsuccessful, you can raise a concern or lodge a complaint with the Washington State Attorney General here.
Nevada Residents: If your appeal is unsuccessful, you can raise a concern or lodge a complaint with the State of Nevada Attorney General here.

Verification of Consumer Health Data Rights Requests
If you submit a request, we typically must verify your identity to ensure that you have the right to make that request, reduce fraud, and to ensure the security of Consumer Health Data. If an agent is submitting the request on your behalf, we reserve the right to validate the agent’s authority to act on your behalf.
We may require that you match Consumer Health Data we have on file in order to adequately verify your identity. If you have an account, we may require that you log into the account to submit the request as part of the verification process. We may not grant access to certain Consumer Health Data to you if prohibited by law.
